Authorization to Obtain Business Credit Report

All information provided, including title/position, business name, type of business entity, state of organization, principal place of business, and authorized person’s name, has been input through the Downstream Marketplace platform application process.

By selecting the authorization button on the form, the user grants authorization for a Credit Reporting Agency of Downstream Systems Inc.'s choice, such as Experian, to obtain a business credit report for the stated business. This report will be used for the purpose of evaluating creditworthiness and obtaining financing for monthly net terms with Downstream.

The user confirms that they are duly authorized by the business to grant this authorization and that all information provided is true and accurate to the best of their knowledge.
